ThriveCare Clinic is a community-based healthcare initiative in Tanzania offering long-term, post-surgical, and chronic disease management services, with a focus on elderly care and home-based recovery. We address the critical gap in care that exists after hospital discharge — especially for patients who cannot afford prolonged hospital stays or require extended care in a supportive environment.
By combining medical supervision, nursing support, physiotherapy, and caregiver empowerment, ThriveCare reduces hospital readmissions, supports family caregivers, and promotes patient dignity. Our hybrid model includes in-clinic admission, home visits, mobile follow-ups, and a digital care companion app.
With a growing need for transitional care in underserved areas, we aim to scale across Tanzania and East Africa, positioning ThriveCare as a trusted health partner for aging populations and chronic disease support.
